We provide medical assessments and documentation for students requiring academic accommodations related to medical or mental health conditions.

This includes forms for post-secondary institutions and other educational programs.

Scope of Service

We complete medical documentation for:

• Academic accommodation forms

• Exam deferral or missed assessment documentation

• Accessibility services verification

• Other medical notes related to  academic impact

Process

1. Appointment Booking
Book a “Student Form Appointment” 

2. Form Submission
Provide the required form in advance where possible.

3. Clinical Assessment
The physician will review relevant medical history and current symptoms 

4. Documentation
Forms are typically completed within 1-5 days, depending on complexity. 

Required Information

Please bring:

• The required school form

• Any relevant supporting documentation (e.g., prior assessments, reports)

• A summary of current symptoms or functional concerns

Clinical Considerations

Medical documentation is completed based on clinical assessment and available information.

For accommodation forms, an established diagnosis and supporting medical documentation are required (e.g., prior assessment by a physician, psychologist, or specialist).

Appointments

Appointments can be booked through the clinic.

If a diagnosis has not yet been established, a separate clinical assessment and referral may be recommended before documentation can be completed.

Fees

Medical forms and related documentation are generally not covered by MSP.

Private fees may apply. This will be reviewed with you prior to completion.
